GMS Social Media Policy A new "Social Media" policy is being implemented. Please read the policy and add it to your personal "Quality Assurance Manual." We are requiring "Acknowledgement of Recipt" form to be signed and returned to Guthrie via mail or dropped off at any Guthrie center. New Arizona 2010 Tax Rates and Withholding Beginning on July 1, 2010, the State of Arizona will change the process by which state withholding is determined. In short, the state will be using a percentage of your taxable gross wages, not your Federal withholding, to determine your State withholding. This means a very different set of percentage rates will be used for the process. Below is an Arizona REVISED FORM A-4 which must be completed and returned to Guthrie Mainstream Services no later than June 1, 2010. Also included is an instruction sheet to help you make the conversion to the new percentages. For those who previously relied on their Federal election to be exempt from Arizona withholding, please note that a Revised Form will still be necessary. Pay special attention to the instructions.
