How to Get Started

Do you think your loved one might qualify for special care?

Here is how you would get started with the process to receive services through DDD.
Application for services may be made at the Division office nearest you. To find an local office near you, visit us on-line at www.azdes.gov/ddd, select the Contact Us bar on the left and it will take you to our automated DES Office Locator feature. You may also submit a referral at www.azdes.gov/ddd and someone will contact you. You may call a local office and ask to speak with someone about an intake interview. Upon making an appointment, a Support Coordinator or Intake Worker will meet with you, review eligibility requirements and the documentation required, discuss your needs and possible support and help you complete the necessary paperwork. You can visit DDD’s website at www.azdes.gov/ddd for information about the documentation needed and other further information.

Do you have a loved one who qualifies for ALTCS services?

We appreciate your interest in our services. While we would love to provide services to all individuals in need, this is beyond our means. If you would like to complete a new member referral, we will do our best to provide you services. However, we ask you continue seeking services through other avenues so you receive the services you require.

It is difficult to staff enough providers to work with the large need in the community. We encourage families to think about family, friends, neighbors, church members or other contacts who might be available to provide services for your family. GMS offers training to certify individuals to work with your loved ones.

When thinking about possible providers to assist your family, they must meet the following criteria.

- Must successfully complete an interview with GMS HR department
- Must have a desire and interest in working with people with disabilities or special needs.
- Must be at least 18 years of age.
- Must have a valid Arizona state-issued I.D.
- Must be able to attain a fingerprint clearance card and pass a criminal background screening. * (can be fingerprinted during training)
- Must be able to complete all training requirements (ie: Article 9, First Aid & CPR, ect)

Once you have found someone who might be a good fit for your family, visit the “Application Portion” of our website. After the potential provider has completed an application, please turn it into any of our locations and indicate they would like to come on to work with a possible new member on question number six.

The process will then begin.
