The following benefits are available for all Guthrie Mainstream Services’ employees throughout the year:

• Verizon: 19% discount and specials (contact Verizon)
• Paid Sick Time (PST): no minimum of hours billed required; PST accrues based on hours worked and is available to use following the completion of your first 90 days of employment.

Employees must consistently work 30 hours or more per week to enroll in the following benefits:

• United HealthCare (UHC) Medical & Group Life
• UHC $25,000 Basic Life/AD&D
• UHC Dental • UHC Vision • AFLAC insurance for Critical Illness, Cancer, Accident, and/or Disability • LegalShield coverage for Legal and/or Identify Theft • 401K

Coverage would become effective the first of the month following 60 days of employment or within 30 days of a Qualifying Life Event*.

Employees must consistently work 20 hours or more per week to enroll in the following benefits:

• UHC Dental • UHC Vision • AFLAC insurance for Critical Illness, Cancer, Accident, and/or Disability • LegalShield coverage for Legal and/or Identify Theft

Coverage would become effective the first of the month following 60 days of employment or within 30 days of a Qualifying Life Event*.

*A Qualifying Life Event can be a change in marital status, change in number of dependents, change in employment status, change in dependent’s eligibility, Medicare or Medicaid entitlement, loss of other coverage, or qualified Medical Child Support Court Order.

Open Enrollment is held during the months of October to November. GMS’s annual benefit year begins on December 1st.

A 401K benefit package is available for all employees who meet all the following qualifications:

1. Complete one year of service as GMS employee
2. Minimum 21 years of age

Considered a full-time employee or will work 1000 hours or more in a calendar year
